Osteoarthritis of the Hand: is Balneotherapy useful?

Hand osteoarthritis (HOA) is the second more prevalent Osteoarthritis (OA) joint localization, after knee, accounting for approximately 23.7% of the total prevalent cases of OA in 2019 and its prevalence increased by 91.84% from 1990 to 2019. Clinically, HOA is characterized by pain, stiffness, functional impairment and occurrence of Heberden and Bouchard nodes, deformities and signs of inflammation, particularly frequent in the subset of erosive hand osteoarthritis (EHOA). Thus, HOA is actually recognized as a possible cause of disability affecting everyday life and determining a reduced participation in social and work activities, other than a poor quality of life (QoL).
Despite its high prevalence and its impact on QoL, for a long time HOA has received little attention compared to other OA joint localizations, as knee or hip. According to the 2018 update of the European League Against Rheumatism (EULAR) recommendations, the optimal management of HOA usually requires a multidisciplinary approach with a combination of non-pharmacological and pharmacological treatments. Among the non-pharmacological strategies, this guideline did not provide any indication about the use of Balneotherapy (BT) as complementary intervention into the therapeutic schedule for patients with HOA because of the rarity of randomized controlled clinical trials.

However, previous literature has suggested the effectiveness of BT in OA, and although limited, also in HOA. Indeed, in the last years, a number of randomized, controlled or observational studies were published in this field. The obtained results showed a positive effect of BT on pain, function, hand strength, and Qol. The long-term studies demonstrated that the clinical efficacy lasted over time, after the treatment. Moreover, the various studies have also demonstrated the safety of BT treatment for patients with HOA, resulting in a limited number of mild adverse effects. These findings highlight the potential therapeutic efficacy of BT interventions and support their inclusion as a complementary approach in managing HOA and preventing its progression.

In this presentation the Author shows the clinical and radiological aspects of HOA and its different phenotypes, discuss the role of pharmacological and non pharmacological approaches, analyzes recent evidence on BT in HOA and suggests the appropriate modality of balneotherapic treatment.

Work Experience
From 1989 until February 2023, she held the position of First Level Manager at the Complex Operational Unit of Rheumatology at the “Le Scotte” Polyclinic in Siena. From 2004 to February 2023, she was also in charge of a Highly Specialized role in the diagnosis and treatment of degenerative rheumatic diseases at the Azienda Ospedaliera Universitaria Senese.
She carried out clinical assistance and both clinical and basic research in rheumatology and medical hydrology. In particular, she focused on the evaluation of chondrocyte cultures to investigate new pathogenetic mechanisms and therapeutic approaches in osteoarthritis and other diseases involving joint cartilage damage. More recently, she conducted studies on the role of microRNAs in the pathogenesis of osteoarthritis.
Her key areas of interest include clinical research on osteoarthritis and the use of balneotherapy in various rheumatic diseases.
